What Happens?
MEDICAL HISTORY
You fill out a medical history form. As well as personal contact details, we ask for information about your general health and any medication you are currently on. This is because some illnesses can increase the risk of gum disease and tooth decay. The information you provide helps us tailor your preventative dental care and a proposed treatment plan, so we can provide you with the high-quality dental care best suited to your needs.
